How much higher would it be than a 0 rail off the comb?

Are you concerned that a 20 MOA rail will encumber your cheek weld?

Yes I am. Starting out with a SS I ordered for a CZ 452 American.  Then a M70 target and Ruger 77. Never did like building up combs, makes a mess with tape glue on wook. Come to think of it, the Unertl never bothered me that much on the 70 at 1000 yards.

It would depend on the design. in most cases it should not alter or it would lower the weld point. The back of the scope is well behind the pivot point to raise the front of the mount.
Think of it like a seesaw.

I'm not concerned about enough SS MOA adjustment. If I use a 20MOA rail on the 452 I can stick my Leupold on too and get more elevation for it.
 
The way I see it the front end of the scope would have to be lowered to get more elevation, thereby raising the rear of the scope.  That's the way a Unertl works.
 
The karsten is is a good idea, but I won't drill holes through these type stocks.

The 20 MOA rail shouldn't effect your cheek weld much, or any for that matter.
