I'm sure everyone has seen Chris Fallow's footage of great whites breaching in False Bay, S. Africa.  He also has some pictures of thresher sharks breaching but as far as I know, know one has seen a mako do it.
 
This particular mako was 8 feet long and boat was 25 feet long, that's a large portion of the boat that they could no longer utilize.  Pretty wild stuff!
